Participation in March of Dimes Canada programs requires consent to the terms below. Please review carefully.


Virtual Program Disclaimer - SkillingUp is an online program and may involve occasional technical issues. Staff and volunteers will support you as needed.


Online Etiquette - Participants may use the chat function to communicate. Comments are saved and visible to instructors. Disrespectful language—including obscenities, insults, or slurs—will result in removal from the program.


Privacy Reminder - Do not share personal details such as your email, phone number, home address, or health information.


Third-Party Resources - While resource sharing is encouraged, March of Dimes Canada is not responsible for external content or privacy practices.




Participant Waiver and Release*


By agreeing below, I acknowledge:

• I am participating in the program with knowledge of the risks explained above.


• I accept the risks explained above.


• Participation in the program is voluntary.


• I can withdraw from the program at any time.


• March of Dimes Canada is not responsible for any injury, illness, disability, death, or property damage that happens when I participate in the program.


• I or my heir or executors will not sue or claim compensation from March of Dimes Canada for any injury, illness, disability, death or property damage that happens when I participate in the program.

Important: Participants under the age of 16 must have this form reviewed and completed by a parent or legal guardian on their behalf.

March of Dimes Canada (MODC) collects, uses, and manages your personal information to administer the SkillingUp Program, deliver training and supports, communicate with you, provide accommodations, evaluate and improve our services, and meet reporting and funding requirements. Your information is accessible only to authorized MODC staff and volunteers who require it to deliver the program. Where possible, information reported to funders is provided in aggregate or de-identified form and does not identify you personally. Your personal information is managed in accordance with applicable privacy legislation and MODC’s Privacy Policy. If you have questions about how your personal information is managed, or wish to exercise your privacy rights, please contact the Privacy Office at privacy@marchofdimes.ca

Consent to Participate
By checking the box below, I acknowledge that I have read and understood this Privacy Notice, understand how my personal information will be collected, used, and disclosed for the purposes of administering the SkillingUp Program, understand that participation is voluntary and that I may withdraw from the program at any time, agree to participate in virtual programming and follow participant expectations, and acknowledge the Participant Waiver and Release.

Where applicable, I authorize March of Dimes Canada to obtain information from and/or share relevant information about my participation, progress, supports, accommodations, and program outcomes with my referring organization, designated family member or support person, assigned mentor, or other organizations involved in supporting my participation in the SkillingUp Program, solely for the purpose of coordinating services, facilitating mentorship, and supporting my successful participation. I understand that this authorization is voluntary, may be withdrawn at any time by contacting MODC, and does not apply to aggregated or de-identified information used for program reporting or evaluation.
